Charles! One of the major reasons for C-sections today is fear of
lawsuits! It may now be the #1 reason, too. Aside from that - 20,000 years
ago, the home town breeding population would most likely contain people
much like you and you would produce children of the relatively correct size
for the people they would grow up to be. Hip and newborn head diameter
would be matched up without too much fuss. Yes, women still died from
breech births and other forms of poor presentation but marriage/mating
(whatever) in the US now brings together people who would never have found
each other 20,000 years ago. And if some women died in childbrith because
the baby's head was too large to deliver, her genes for narrow hips or big
headed babies would still be carried by a sibling. Even "bad" traits are
hard to kill off.
