The former. Each pixel is based to the values of the pixels in the 7x7
area centred on that pixel. The result for that pixel is the weighted
sum of the 49 pixels, (the weights being those of the kernel). You do
this for every pixel individually.

Edge effects are a bit more complex no matter the size of the image.
In the 7x7 case, there simply aren't 3 columns of pixels to the left
of the leftmost column of pixels. I just crop the edges.
