Adapter, thermometer (or tubing), teflon; O-ring seal
(Buna, Viton, Kalrez or other); you can get them with
an optional O-ring seal on the body, too.

and many other places: Ace Glass, ChemGlass, etc.
Teflon is pricey. You can use a glass reducing adapter to
get down to 24/40 or 14/20 and use a smaller teflon adapter.
A good machine shop can make one for you fairly easily.
