You need a .MODEL statement defining the MOSFET.
Placing "0" for "?" for the "bulk" terminal works on this case since
it's Nmos. But be cautious with an extractor that can't find
"bulk"... add PMOS and the "bulk" can usually be arbitrarily
connected.
