Everybody pays and only some collect. The 3% is paid through out your
entire working life.
That being said, not the system isn't self-sustained. It is going to
run in the red eventually as the baby boomers retire. Still in terms
of money in to care provided it is a fairly efficient system. It has
an overhead cost of something like 3% which isn't too bad when you
compare it to the 20% or so of private insurance.
