I've occasionally seen that sort of thing certainly with XP and maybe even W98.

Get yourself a copy of cacheman (cache manager). It's free. It allows you to
tweak the icon cache value easily. I imagine that's how I fixed it but it was
ages back so I've forgotten the details.
