There isn't a lot else to hit besides the ground so unless you hit another
airplane in flight, the crash will involve hitting the ground AKA "a
landing".

There are a very few that involve hitting terrain while cruising. Also
some that involve mechanical failures while cruising. But these are very
rare.

NO.

CFIT (controlled flight into terrain) is a regular and stubbornly persistent cause
of accidents.

Plenty of mechanical failures kill too.
