there would be much more possibilities to help you
if you could give some details on the modality and
contrast injection protocol. I suppose (could be
plain wrong) that you are dealing with contrasted
x-ray projection images and that "black" means the
attenuation of the contrast agent.

Then you should be aware that the total amount of
contrast agent in some region of interest does _not_
correspond to the blood flow into this area! If you
assume this, then your estimation will depend on the
moment in time when you measure (i.e. the frame you
use) and the total endovascular lumen volume in this
region of interest as well as the total amount of
contrast applied. However, the change of the attenuation
between subsequent frames is then proportional to the
blood flow (assuming that the contrast concentration in
the feeds to all considered regions of interest is
identical, you might need to include feeding vessels
and draining veins into your region of interest to achieve
this).
