What a superb and comprehensive answer -- at least IMHO having dug out
bits of information in the period since I asked the question.
You may be aware of the uninformative drivel one finds about this
subject on the web; and I'm not aware of an up to date textbook that
covers the topic properly.
Oddly enough, after giving up on the wider web, I did find something
useful on uk.comp.os.linux.
