What makes you think Japanese people who want to look up mizu are confused?
I am sure that there is not even one Japanese person who is capable of using a "kokugo jiten" dictionary yet who doesn't know what "mizu" means. I really doubt if there is even one non-native speaker of Japanese in the entire world who might think of looking up words in a kokugo jiten who doesn't know the meaning of "mizu". The dictionary in question is intended for native speakers who want to look up the meaning of words in their own language. An extended entry for "mizu" would be superfluous.
