I found the following photo on flickr:

The photographer claims "A carved wall in a Chinese restaurant in Shinsaibashi, Osaka. They're all the same character (meaning luck, wealth or blessing), but in styles from differing eras."
Now, I happened to have a copy of a character dictionary nearby:

and I don't see anything similar to the characters at positions [1,3] or [2,2] in the character dictionary.
Are these really forms of "fuku"?
Yes, that's right, all are character variants for what you said. In some books, you see the 百福全圖 , and there is one for 壽 or longevity as well. That, unsurprisingly is called a 百壽全圖.

I don't think that your dictionary will list them all. Maybe the commoner variants.
