There may be regional differences, but mostly I feel it is a prestige form, used by people who speak elegantly and well. I seem to hear it less now than when I first came to Japan thirteen years ago, and I wonder if perhaps it is on the wane.

日本語の「標準的」な発音では、「ガ」行音は、語頭では「g」、
語中、語尾では「ng」と発音することになっています。
これは、日本語を極力統一するために、東京・山の手の発音を
日本語の「標準的」な発音である と定めたところから来ている
もので、アクセントもまた、東京・山の手のアクセントが「標準的」
なものとされています、
従って、人前で話しをする人々、例えば、アナウンサー、俳優
（映画の俳優を含む）、歌手など、（歌舞伎役者や演芸人は除く）
は、この「標準的」な発音を行うことが期待され、また、実際に
そのような訓練され、矯正されてきました（これは、バートが既に
指摘していますが）。
時にはこの「標準的」な発音ができなかったために、その仕事に
就けなかった、あるいはその仕事を失ったことさえあるようです。
ただ、現在、この訓練を経ていない、素人（同然）の人々（例えば、
料理人、コメンテーター、芸人・・・）がテレビなどに出演するように
なってから、この建前が崩れている こともまた事実です。
が、それを on the wane とまで言ってしまってよいのかどうか、
私には少なからぬ疑問が残ります。
