Straight off the cuff I can give you Slovenian:
Cases: N G D A L I V
Numbers: Jednina Dvojina Mnoz^ina
Genders: 3 with more than one declension paradigm each
Both nouns and adjectives have three numbers
IIRC, at least one of the Sorbian languages still has a live dual.
