Only because the (almost inaudible) distinction is
fetishized in Britain. Most people do not (almost,
_cannot_) hear the difference.
You're missing the important fact that much conversation is
face to face. Combine the slight acoustic difference with
the very different appearance of the mouth, and it's pretty
easy to tell when a speaker uses [f] for [T]. (I speak from
experience: I've a Brazilian colleague who makes this
substitution.) The same of course goes for [v] and [D], but
in that case there's less need for the visual cue.
