editorial policy question

What exactly are the ethics of a referee contacting an author directly,
instead of communicating indirectly through the referee? I'm aware that
this is done sometimes and is even desirable sometimes, but I'm not
absolutely certain what the explicit policy is of journals and their
editors. I'm sure people have a lot of opinions about what is reasonable
and about what makes sense, but what I really need is to hear from people
who have served as editors of journals about the official policies of
their journals. Also, this is something that might differ from one journal
to another.
This question came up in a discusion on sci.lang and we eventually decided
that the only way to settle it was to ask about it on sci.math, which I
agreed to do.
