No offense, but I'm trying to avoid digression into what the policy
OUGHT to be, since the question it is the purpose of this thread to
resolve is what the policy IS at various journals. Also, I think editors
are better prepared to answer that question, so I prefer answers from
editors.
A possibly different policy might hold for books. No one has commented
on that yet.
