What you describe could possibly be "labile" hypertension or even just
white-coat hypertension. Though the former can lead to hypertensive
heart disease, the latter typically does not.

Tough question. Given your family history, imo, one should be more
aggressive and favor starting something cardioprotective like ramipril
and hope (pun intended) that you will tolerate it.
