what are palpitations?

I know what a fast heart rate is and a slow heart rate..
But what are palpitations?
Medical dictionary refers to it as a "subjective" perception of the
heart beat either as loud, slow, fast, or skipping or adding beats..
Can it be seen by a second person? or is it purely subjective. Ie.,
can a second party say oh you are having palpitations even without
being told? As well what would cause a "loud" heartbeat enough to be
heard, is it a larger cardiac output..
