Need Expert Medical Opinion...

Hello,
I was really touched by reading an article today about a 20 year old
girl dying from a potassium issue after surgery, and I really need
someone more knowledgeable than me to answer a few questions if
possible. Article is below:

My Questions:
-Why would you ever after surgery check blood pressure by putting a
line in a vien? Isn't blood pressure always checked by cuff?
-Why would anyone give a potassium by IV for a "slight imbalance"
considering the risks? Wouldn't this only be done in an emergency
situation?
-How does a central line "accidentally" get up into the heart??
-And finally, is the coroner right in that it was a "freak accident",
or is he simply trying to cover up someone elses grave negligence?
