Sorry for not being clear. My comment was that the diabetic group tested was
pretty healthy, in a cardiovascular sense--the last of your three
possibilities. It is apparently possible to be a type-2 diabetic, have a low
LDL, and be relatively safe from heart disease.
There does seem to be some protection from the Lipitor, but the composite
endpoint chosen and the population chosen certainly didn't result in the
kind of relative or absolute risk reduction I would have expected.
I've read only the abstract, so my comments aren't worth much.

What really seemed to happen in this study (as is happening in many such trials of lipid lowering these days), is that there were a lot of crossovers and dropouts. This combined with a confidence interval that included the relative risks seem in the vast majority of trials of statins probably explains why this trial looks like an outlier.
