Why should the doctor mention something that has very little chance of
success?
The motivation to lose weight and be healthier and change
one's lifestyle has to be greater than the desire to stay the status
quo and that is why for most weight loss is temporary at best.
I think the main problem is that when you restrict your calorie
intake, your metabolism slows down. Accordingly, the weight loss can
only be maintained through strict calorie restriction.

also smoke, drink alcohol to excess
Smoking and alcoholism are addictions. If overweight or obese people
were addicted to food, I could see the analogy. However, most are
not.
not wear seat belts and ride on
motorcycles without helmets too.
This does not seem like a good analogy to me. Wearing a seat belt or
putting on a helmet is a simple thing to do. Losing weight and
keeping it off is not.
