You will notice that I belong to various dental associations. None of these
groups set any fees, neither maximum nor minimum. They have to say of fees.
They are very careful never to suggest a fee for any procedure.

And insurance companies ONLY negotiate with employers to create dental
plans. They only negotiate with the people (or companies) who are going to
pay the premium. The employer decides how much premium they can afford to
pay per employee. The insurance carrier then suggests a package of coverage
for that level of premium. Sometimes, to meat the limitation of the premium
which the employer can afford, certain procedures are left off the coverage
plan. Other times, to keep the premium levels lower, but still offer
coverage for a wide assortment of procedures, all the maximum fees are
artificially set at a lower point. The employer does the best he/she can
for the dollars available to spend on coverage, and the insurance carrier
does the best they can while guaranteeing a profit for their stock-holders.

Don't kid yourself, dental associations have zero input on fee levels.
Dental offices set fee levels by determining how much it costs the office to
perform a certain procedure then adding 20-50% for personal profit. Since
rent, utilities, supplies, lab fees, payroll, insurance, taxes, etc. are
pretty standard for any region, the dental fees end up being moderately
close to same level.
