Martin,
I've worked on cases similar to yours with a prosthodontist. Some of them had
some ortho to improve things for the prosthetics. If you only do the front 6 it
may look like you have no back teeth with your narrow arch and wide smile. You
might have to include the bicuspids. One way you can get a good idea what can be
achieved before dropping a lot of money is to have a diagnostic waxup. That will
give you and the practitioner a good idea of what might be achieved, and how. A
little work on the lowers might be required to optimize the uppers. Throw in a
touch of reshaping some of the gums and things could be looking good.
If I was waxing up the case, I would be making all the teeth a little wider
keeping them in proportion. Check your email for my first attempt at using a
photo software on your teeth. It is very rough sketch to give you some idea of
what I am talking about.
