I am no expert, but I like the Meyer S Leonard technique: First, gain
access to the sides of the tooth using a flap or bur, what ever. Grab
the tooth with beaks of the forceps and tease them down as far apically
as they will go.
How, PUSH on the tooth as if you were going to force it up into the
brain or down and out the other end of the lower jaw. Hold for eight to
ten seconds. Release, rest, do it again with an apically directed force
and with a twisting force as well. Hold for another eight to ten.
Do this until you feel the tooth coming loose (subluxation). Now, if you
use any fancy technique other than keeping this up until the bone is
sufficiently expanded), it will be easier to extract the broken
fragments, as they have been previously subluxated. Often an endo
explorer will tease a root tip out after the above.
