Leaving a root tip (as in doing the extraction at all) is a surgical judgement. I'm assuming this was a permanent and not a primary tooth. It is of course preferable to get the entire root out. But sometimes the root is near a structure (say, the inferior alveolar nerve or perhaps the floor of the sinus) and the surgeon judges that getting it out poses a risk of bigger problems. Sometimes these root tips are left in (I've seen it many times) and they seldom cause any problems, esp. if they are deep in the bone. If the root is right below the gum however it should be removed.
