I haven't.
I overlooked the home hospice care option.
Most people are programed to think of hospice as somewhere other than
home.

Home hospice is another likely option but the subject will put anything
like this off as long as possible.
It is unlikely anything will be done until the subject becomes
incapacitated and requires routine care.
