Nasopharyngeal Carcinoma, few questions

I'm trying to provide support for a friend (southern Chinese parents but he's born in the USA). A diagnosis has been made by biopsy but staging is not complete. He and his wife just learned about it so they're still in those initial awful days after diagnosis. My past experience with cancer is my wife's breast cancer.
All of the papers I read report the results of treatment on locally advanced cancer. Why only locally advanced? Is there no T1 nasopharyngeal cancer?
What's the current gold standard treatment? I see both radiotherapy-only and concurrent chemoradiotherapy mentioned.
How do they determine nodal status? This is important in breast cancer, so I assume the same goes for NP too.
