Not necessarily, its complicated but I'll try and keep it simple:
Stage IV is distant mets like in lung or brain, bone or liver. (M1)
Stage IV also when the lymph nodes are N3 meaning >6cm in size or in
the supraclav. fossa (ie down to your shoulders)
Stage IV also T4 tumor with any node stage.
Now you know why your staging according to TNM would be useful to know.
But its also a reason why something your oncologist said makes me think
you may be stage IV. He said the PET scan was not urgent because it
would not affect treatment, so you (probably) have N3 nodes..?
At first I thought there might be cost reasons..

Normal treatment would be about 15min, yes.
IMRT: Steph is doing it, ask him how long treatment takes.

If the simulation is done carefully, then high quality treatment is

Slightly, its just fluoroscopy (like when you get a barium
swallow/stomach x-ray).
