She's wrong. The mediastinal nodes are very large, but the patient also
has intrapulmonary mets.

They made CT in november 2004 and found only the enlaged lymph nodes
outside his left kidney.
"outside his left kidney" is not a defined anatomical region.
Were the LNs around the kidney hilus, along the renal veins or within
the paraaortal LN group? Did the surgeon remove the nodes, were they
positive histologically? Did the tumor grown INTO the renal vein?
Whatever, the patient is now stage IV.
