I am having hives. Herx or Allergic reaction?

How do I tell if rashes and hives are an allergic reaction or due to herxheimer reactions?

I started taking antibiotics on August 14, 2005.
I took 6 weeks doxycycline 400mg/day,
then followed by amoxicillin 4500mg/day.

On Oct. 4 (the 10th day of taking amoxicillin) I started getting red rash, especially at places where I normally have small pimples, like on my arms. So I got all red spots on skin. It didn't itch much.
This lasted a few days, but from about Oct. 8/9 it started to diminish.

But on Oct. 14 I got hives, it itches a lot and I have an uncomfortable feeling in chest and stomach, and I feel a bit cold, a bit like I'm having the flu.

From two days ago, on Oct. 16, I didn't take amoxicillin, because I thought this could be an allergic reaction. I have been taking the amoxicillin for 3 weeks. I still have the hives, Sunday and Monday it was the worst, today it seems for the first day a bit less severe.
