Again...what is the difference...between a "confirmed diagnosis" and a
"diagnosis"?

My impression has always been that once a diagnosis has been reached,
then appropriate treatment must be provided.

Not necessarily, and it depends on the nature and course of the illness and
the nature of the treatment. A probable or presumptive diagnosis could be
reached; perhaps treatment would be started, but it would depend on a number
of things including the treatment, the health of the patient, the rapidity
of the course of the disease, etc. The treatment could be considered too
costly financially or to the patient's health without a firm (confirmed with
additional information) diagnosis. The patient might not have
insurance.................how many times is that the reason people don't get
treated?

It seems that IDSA is stating that lab testing is a REQUIREMENT for the
confirmation of LD diagnosis(other than EM rash presentation). Can the doc
treat without confirmation? Well, after reading these guidelines, how many
would? And would insurance pay without the lab confirmation? Knowing that
insurance would require confirmation, would a doc treat without the labs?

Google 'diagnosis' and 'confirm diagnosis' to get a feel for the
definitions.
