What makes you think this is Lyme disease, rather than an oral infection?
All of those symptoms are common in people with adrenal insufficiency in response to long lasting infections, when they go into acute phases.

In December a small pain started in his left upper/middle back. The
first day, rapid fasciculation (muscle twitching) in his eyelid for
several hours, then disappeared.
Within 4 days, the original back pain radiated out to the front of his
chest. Started to then have muscle fasciculation on his left & right
triceps.
Doctors said it was neuropathic.
MRI was taken of thoracic spine, all clear.
Blood tests were taken, all clear.
Neurological exam showed no loss in muscle strength.
Ruled out ALS or MS.
January - Doctor Oked him lifting weights again. Started to lift
weights and pain lessened, but still was there. Fasciculation went
away.
We left for a trip to New Zealand February 23rd. Back/chest pain was
still evident, but only feels it when sneezes or breathes deeply. No
more fasciculation.
New Zealand is heavy activity, but he seems healthy.
All of a sudden, his neck goes stiff, cannot move it. Starts feeling
rundown and weak. Has diarrhea for 2 days. Wakes up several nites
with tooth pain.
Neck stiffness goes away in 4 days.
No fever, blood tests normal.
Since this day (over 3 weeks ago), he has been feeling rundown and
tired in waves. It comes and goes with no relationship to activity or
to anything. He says it feels like he is coming down with the flu
when it happens. Body aches, rundown etc. Then it suddenly goes away
and he feels perfectly normal.
No loss in strength.
Blood tests are all clear. (checked thyroid, blood sugar, Wbcels, etc)
Doctor here says it is mental since there is no conclusive evidence.
This is bull***, it is not in his head, trust me.
We self-medicated. We have some Levaquin 750 mgs here. He took it
and his rundown symptoms went away, but he had to stop taking it
because the side effects were too harsh (testicular pain, severe joint
pain). He felt as if the antibiotics worked though when he was on
them. Original back/chest pain is still the same.
He was on Levaquin for 3 days, stopped for 2 and now his rundown and
tired symptoms returned even worse. His joints in his hands and feet
ache. Muscle fasciculation has returned.
Possibilities include Lyme Disease? (he hunts in Wisconsin)
All tests are normal for him though.
Doctor gave us a prescription of Doxycycline in case it is lyme
disease, but only 7 days worth. He just started yesterday with 200
mgs a day.
We are going to Hawaii next week and to the doctor, but are afraid
they will find nothing. Every time they take his blood, it is fine.
