English Abstract :
This retrospective descriptive study is aimed to evaluate ?Prevention and
Control of Thalassemia Program? in Buddhachinaraj Hospital during 1999 - 2002.
Four thousand, eight hundred and twenty six pregnant women were screened and
1330 were positive (27.6%) and almost all of this group were proved to be ?the
thalassemia trait?. In thalassemia trait group, thirty percent were found to be
a late attendants (first visit later than 20 week?s gestation). Most
thalassemia trait was hemoglobin E trait and homozygous Hb E (93.3 %) but low
rate of ?-Thalassemia trait (3.8%). Only approximately half of the husbands of
the positive cases were given counseling and had blood tests. About one third
were positive and the distribution of trait group were similar to the positive
pregnant women group. It was disclosed that 16 couples were at risk which
represented 1.2 percent of the total positive cases. Only three pregnant women
underwent cordocentesis. Four main problems were addressed; a low rate of
husband tested, a low rate of cordocentesis in couples at risk group, a high
rate of late ANC and no ?-thalassemia were screened. It is highly recommended
that all staff should improve the counseling process. After the year 2002, the
Health Research Institute, Naresuan University was contacted for more screening
of ?-thalassemia. Government should support the budget for the antenatal
diagnosis.
