UTI advice

Please excuse me if I have posted in the wrong group and feel free to
direct me to the correct group.

After a video urodynamics study (about 1 year ago), it was found that
I have a very poorly functioning detrusor which results in residual
volumes on voiding of 350-400 mL. I can void ~ 200 mL of urine when
my bladder is holding ~600 mL.

I was told the problem is not nuerogenic in origin and was told
(rather inadequately) "I was born with it". A bladder neck incision
is considered not to be an option as I beleive this requires more
detrusor activity then I have to be effective. I am male, mid 30s

Anyhow, I have been performing intermittent catheterization 4-6 times
a day. Of course now I am battling UTIs. I have abandoned
trimethoprim prophylaxis which was ineffective and am now treating
UTIs on a case by case basis.

I would love to hear from anyone who can give advice on decreasing the
frequnecy of my UTIs (currently being hit at one a month). Cultures
show them to be mainly E-Coli in origin (one was a bacteria more
associated with skin infection, but I can't remember it's name)

If I try Cranberry capusles, how many should I take a day
?-Ancedotally-how effective are they ?

Pre catheter insertion, I wipe the end of the penis with an alcohol
swab, should I use a more thorough wash (with for example some form of
iodine solution-this is not always very convenient if I have to use a
public toilet !)

Anything else would be really great.
