Well, any sexual act without the permission of the woman constitutes rape,
agreed.

However, Janice is correct that the one under discussion is not that
uncommon, and has often been practiced in many times and places as a) a
means of birth control or b) as a means of having premarital sex while
maintaining the woman's technical virginity. Of course, it is also practiced
more often for purely erotic reasons.

Even in the U.S., where there is a strong cultural taboo, studies have
estimated that it has been engaged in at some time by 20% to 35% of the
heterosexual population and up to 80% of the male homosexual population.
(Yeah, that's right, probably only about 80% of American male homosexuals
have ever done it that way, and it is estimated that only about 2 out of 3
gay couples engage in it regularly.) There are differences in sexual
practices between those with different sexual orientations but, as usual,
not as great as some people would think. And, again as usual, the United
States scores towards the prudish end of the scale when compared to the
world at large, where heterosexual frequencies are closer to 40% to 50%.
