Phacoemulsification w/ lens implant

Does anyone know of a good place to find ophthalmology sample reports? The ones at MT Desk aren't helping me with this, which is probably pretty basic for someone familiar with ophthalmology, but it's giving me fits!! I have the Stedman's Word Book for ophthalmology, but I'm still getting nowhere, and I'm getting so tired of cringing every time I pull up an opthalmology op note. It's really getting embarrassing!
