Wow - pretty nervy list if you ask me.

#1 is appropriate.

#11, #12 and #13 are appropriate only if this information isn't
already detailed in the resume they have (presumably) received. If
they can't extract that information themselves, why should you waste
your time spoonfeeding it to them?

#17 I think is appropriate.

I like #19 - send us your business. LOL

Everything else I think is grossly inappropriate to ask an independent
contractor. Even an employee - if they don't want employees
moonlighting, then make it a requirement of their employment, no need
to get all that information.
