I worked as a clerk in Medical Records ages ago. This was a medical
center in California. I was responsible (among other things) to file
and/or locate old patient charts/records stored in a hospital-rented
storage facility. I remember many records that were more than 10 years
old. We store the old records in alphabetic order.

For each admission date, we assign a new patient chart. All medical
reports, progress notes, labs, *pathology reports*, etc., were filed
inside the patient chart for that specific admission period.

If the patient was admitted for a long time, the patient can have a
very thick chart or even multiple charts for that specific admission
period. If the patient had chronic admissions, he/she had numerous
charts that can occupy a whole library bookshelf.

Perhaps the missing pathology reports in your specific case were in
another chart or charts from different admission dates.
