In spite of everything you just said, I don't see where your typing speed
has a thing to do with what your work is worth. If anything, the faster you
are, the more you can produce in a given hour, which will make him tend
toward paying you less per piece, not more. All of the things that you list
as extras, such as catching mistakes and so on, are really just part of
medical transcription in my opinion, and not "extras" at all.
