That's exactly what prompted me to raise the rates. It just isn't enough.
This part isn't their problem, but basically I needed to take on a second
job, but couldn't because I didn't have time. So I evaluated things, and
really wrote things out over the past few months and discovered that even
without the extras, local rates are higher than what I was getting, all in
all. In fact, I've been posting questions and thinking about this for a few
months, so it's not something I went into lightly. I knew there was a risk,
but thought it was a very low risk. If they go back to doing some things
themselves and pay me a lower rate, I'm perfectly happy with that as it will
allow me to get another job. Even though they tell me pretty frequently I'm
doing a great job and "it's taken years to find somebody like you," they're
considering going with somebody lower. At this point, I'd be happy for them
to let them send work to another company, and have them call me when they're
ready for me to start up again. The guy is wonderful, but he doesn't pay
attention to what he dictates so there are a lot of contradictions and
medication errors that would not be picked up by a company, since he
dictates it clearly (like my Coumadin meaning Neurontin example). Throw in
the "97% accuracy rate" most overseas companies guarantee, and their notes
would be a mess. Nothing dangerous that I can remember, but it certainly
wouldn't instill confidence for doctors to refer patients to him. Honestly,
I don't think I'd fight for this account, but I definitely would negotiate.
After doing the account for 4 years, I can pretty much type out the note
myself after him giving a few key words. There's comfort in familiarity,
but I have been fighting a little burn out so this may be a blessing in
disguise coming up.

This is all great advice you've been given. I would also take a look at
exactly what my takehome pay on this is, what's left for you after taxes
and expenses? Is it enough? Is it enough if you can either start
charging for services you've been giving them for free or if they go
back to doing them themselves? The answers to these questions will
give you some guidance about how hard you should fight, if it all, to
keep this particular account. Good luck to you.
