I hear you! I know there have been times I've done exactly that. You know it's wrong and correct it, but then there's this little nagging "what if I'm wrong" until somewhere later on they make it clear that's what they meant. Of course plenty of Airheads would never catch the discrepancy to start, never mind think they should actually fix it - and some would stand there and argue with you that you're overstepping as well. One reason why I like working for same doctors. After a while, you at least know the types of mistakes they make and are on the look out for them.

This type of thing really, really exhausts me when I'm transcribing. It requires so much more brain power than just transcribing words and sentences. He knows how bad he is, and they pay their bill on time so I guess all I can do is b!tch about it occasionally. LOL
