Say you wish to test a drug. So what you do is run a series of tests
using placebos to test against the patient that are getting the
medicine.

I am wondering what is the justification afterwards given to those
patients that are sick and are being given placebos thinking that it was
medicine.

The justification is given prior to entering. That is part of the
informed consent. The people know going in that they may get a placebo.
