At least for the German Army, real soldiers who had assumed a secondary
role as medic (or rather stretcher-bearer, medics need quite a bit more
training) had a smaller-than-usual red-cross marking on their uniform,
but while wearing that marking they were supposed to be treated just
like normal medics... i.e. not fired upon under any circumstances.
