I suspect you're going to need a piezostage, due to the better
positioning resolution - 10nm (not um) in X,Y and Z.

And yes, I just bring the cleaved end of the fiber into proximity with
the waveguide - not touching, but ~1um (or less) away, as observed
through a microscope.

Getting a good cleave from the fiber takes some practice - you can tell
when you have a good cleave by having a circular spot when you shine the
laser light on a piece of paper. (And the spot should be in line with
the fiber - not off to the side.) I use a Fitel Furukawa S-315 fiber
cleaver. It's manual, but the automatic ones (which would probably due
a better job) are very expensive.

I'd like to polish the cleaved tip of the fiber, but I don't have the
necessary equipment.
