That's quite a specification list! NA=0.45 in a telecentric lens sounds like a 20X microscope objective...which you can get with a 200 mm tube length, pretty close to two of your specs (at least in the visible) but that 3.5 mm FOV is going to be the deal breaker there. I have a couple of _really_expensive_ Mitutoyo IR microscope objectives that would be within a factor of 2 of most of those specs...but they were $10k apiece, back in the day.
