Yes.

Yes. Generally speaking, when we talk about standing waves we're talking
about CW behavior. Of course, that begs the question, since we deal with
non-eternal things in real life. If a pulsed signal has long duration
compared to propagation times up and down the line, then there will be
standing wave for a period of time after the pulse has been on for
"awhile" and not yet turned off.
