The JSC Saturn V does indeed consist of all flight-rated stages, but they
did not all come from a single Saturn V.
The KSC Saturn V second and third stages were flight-rated, but the first
was a test stage.
Another flight-rated first stage wound up in front of the Michoud Assembly
Facility in Louisiana, where it was built.
See the Field Guide to American Spacecraft
<http://aesp.nasa.okstate.edu/fieldguide/> for details. I am not sure if
this site has complete info on the CSM/LMs, though.
