The footage itelf is from the first unmanned Saturn V test launch, (which was Apollo 4) on November 9th, 1967.

I'm not sure which Saturn IB launch the footage of the S-IVB separation is from, but there is footage of the original S-IV six engined Saturn I top stage separating on the same company's "The Mighty Saturns: Saturn I and IB" video, as it's shown on the prevue: http://www.spacecraftfilms.com/saturn1preview.html
