The problem with Apollo 13 was ultimately traced to a thermostatic
switch stuck in the "on" position, with the result that the heater in
one of the O2 tanks was on continuously. With the advantage of
hindsight, might it not have been possible to detect continuous
current flow to the affected heater *before* the explosion?
