Gimli Glider was an incorrect fuel load due to units confusion.
Transat Glider was not following a technical bulletin leading to a fuel
line breaking in one nacelle and the pilots moving fuel around to
rebalance tanks and eventually dumping it all out said broken fuel
line.
But there's still the Ethiopian 767 ditching in 1996 after being
hijacked and running out of fuel. Killed lots of passengers after not
deploying flaps and hitting the water at 130+ knots.
